Personalisation and Self Directed Support

The Perth and Kinross Self Directed Support team provide in house support for clients, families, private sector, health and social care partnership and the 3rd sector,  across all ages and client groups.

This is done through 1 to 1 conversations and group sessions with clients and families, groups and new and existing workers, as well as on going support with locality teams to promote personalisation through the SDS process, to enable choice, flexibility and control.

The Self Directed Support Team and the Learning and Development team work in partnership, striving to continually meet the personal learning needs of individuals and teams by being responsive and exploring personalisation and SDS across all localities.

BEFRIENDING SCHEME FOR CARERS

The Befriending Scheme for carers in Perth & Kinross is a new initiative which hopes to enable communities to support one another and to bridge the gap between the support unpaid carers are currently able to obtain and the support that they need, particularly to have a break.

Social Prescribers

 

Social Prescribing starts with a conversation. It maybe a conversation with a GP or with someone else from the Social Work Team.

Social prescribing has been shown to offer social and practical support to people with a wide range of challenges and health conditions whose needs are not currently being met through existing services.

The overall aim of Social Prescribing is to improve the health and wellbeing of people by signposting and supporting them to access and use community-based activities and provide a different response to those provided by Health and Social Care services.
