What is Social Prescribing?

Social Prescribing starts with a conversation. It may be a conversation with a GP or with someone else from the Social Work Team.

Social prescribing has been shown to offer social and practical support to people with a wide range of challenges and health conditions whose needs are not currently being met through existing services.

The overall aim of Social Prescribing is to improve the health and wellbeing of people by signposting and supporting them to access and use community-based activities and provide a different response to those provided by Health and Social Care services.

 

 

What are Social Prescribers? 

 Social Prescribers support individuals to unpick issues they maybe facing . Putting them in touch with people, activities that provide practical and emotional support. Enabling individuals to take greater control of their health and wellbeing.

Why does it matter? 

Social Prescribing improves outcomes for people by giving more choice and control over their lives and improves sense of belonging when people get involved in their community groups .

Its also effective at targeting the causes of health inequalities and is important facet community centred practice. 

 

 

Individual outcomes for people could include:

Improved self - management of health and wellbeing

Improved quality

of life

Increased skills and abilities
Increased social contact and reduction of social isolation
Improved mental wellbeing

 


 

Introducing Your Local Social Prescribers

Hello,

As a team of 7 Social Prescribers, we offer an alternative approach to non – clinical and traditional statutory services across Perth and Kinross.

Working in partnership with Health and Social Care we provide people with information and advice by signposting to access local community groups, 3rd sector organisation and voluntary opportunities.

We accept referrals from individuals aged 16+ to older adult and will work with each person on a short-term basis. Our aim is to improve people’s health and wellbeing through self-help, empowering people to make informed choices, build confidence, learn new skills, meet new friends, and reconnect with their community.
