Living Well Care Team

A warm welcome to the Living Well Care Team!

Living Well Care is a Care at Home resource established to assist care at home, recruitment, flexibility and to support rural areas.  Here in Perth & Kinross Council, we believe all our journey’s matter!  We aim to develop an ecosystem, where our learning culture across the Health and Social Care Partnership invigorates our people to bring their best selves to participate in co-designing learning and enable the care and support that makes a difference to the people of Perth and Kinross.  We want to continue to grow and create an environment where our people are eager and enthused to learn, and in turn we are able to use this learning to influence and maximise our ability to enable person centred care and support.

Our Continuous Professional Learning Commitment

We will will continue to work with our partners, locally and nationally to influence how we develop our learning offer.

We will continue to listen to our workforce and the people we serve and will continue to create a learning environment for continuous improvement and innovation.

To providing meaningful care and support in Perth and Kinross, we ensure that we evaluate learning, practice and work with our partners to continuously improve.

We are currently maximising our collaborative space locally in Perth and Kinross and beyond to explore our resources, support development and create learning experiences that enable our people to release their potential. As we move forward, we will continue to listen and observe, ask for feedback and evaluate the learning environment. This will influence how we work together as a learning partnership where we nurture and compassionately create a culture that enables us all to flourish and support us to enable the people we serve to live life well.

Our values and commitment to you and the people of Perth and Kinross

We are committed within the Living Well Care Team to implementing an induction framework along with developing and implementing a Continuous Professional Learning framework to ensure that we have a workforce which has the skills, knowledge and values to support those who access long term care at home. Enabling those individuals and their families / carers to have access to support which is locality based, person centred and will enable them to meet their outcomes and live better lives.

The Living Well team's help with my father has been of huge benefit to both him and myself since January. The extra time available for him has boosted his morale and mood considerably and given him the incentive to talk more and communicate about any issues he has, whereas before he was more withdrawn with carers - and me. Every member of the team has been fantastic with him and he enjoys their visits and time spent talking, something some days I do not have the luxury of because of a very stressful job working long hours. I have certainly found this to have benefited his mood on days he has been feeling down or agitated about something. The team has made a huge contribution to his mood and I know he is grateful for their help and company. The service is excellent and very rewarding.
